If you would like to confirm that our services are covered under your insurance plan, we recommend contacting your insurance provider directly using the phone number listed on your insurance card. Your insurer can verify whether your appointment will be reimbursed or applied toward your deductible, which may require out-of-pocket payment until the deductible is met.

Once you are established as a patient, it is your responsibility to notify our office in advance of any changes to your insurance coverage (including changes in plan, provider, or loss of coverage) or updates to your personal information (such as legal name or address). Failure to provide timely notice of insurance changes may result in charges being billed at our self-pay rates for subsequent appointments.

Please note that verification of insurance benefits does not guarantee coverage. If your insurance plan does not cover a service at any time, you will be responsible for any outstanding balance.
